ABOUT solutions make it easy for health systems to find facilities that meet patient criteria, simplifying patient acceptance. Participation is always no-cost and easy—just log in and respond to referral requests.
Instructions for Providers
Step 2
View Referrals
- All active referrals are on the Referral Listing Page.
- If you haven’t responded yet, “Referral Received” will show under Latest Activity.
- Click the patient’s name/MRN for details and to respond.
- Patient info appears at the top of the Sending Response Page.
- To view or print clinical documents, click “Download” under “Discharge Packet Attached” in the Activity Log.
Step 3
Send Responses
- Click on “Add Response.”
- Check the appropriate responses for “Yes”, “Considering” or “No.”
- Once the response is selected, date or reason options will expand and you can select the options for your response
- Send a message to the planner with your response by typing it into the “Note to Hospital” box.
- Click “Send Response” to finalize your selection.
- To revise your response, respond again with your updated answer.

Provider Tips
Stay Informed and Respond Quickly to Referrals
Enable notifications to receive email and/or text alerts when a referral is sent.
- View hospital referrals by logging into the Respond Portal and respond within 30 minutes.
- When a “Yes” response is received, the discharge planner will contact you to finalize placement or service.
- A “Yes” response does not guarantee placement.
- If your facility is not chosen, you’ll receive a “Case Closed” notification via text/email.
- If a patient is placed with your facility, you’ll receive a “Booked” notification via text/email.
